Add destination information to fax email

Description

When a person receives a fax from the gateway there is no indication of the end destination for the fax. This can create problems when people have multiple DID's or extensions that end up in a "general" fax mailbox OR a single person is responsible for distributing.

The attached patches change the fax-process.pl call in extensions.conf to include a destination and adds text and processing for an additional command line option in fax-process.pl.

this looks good but I don't use fax-process.pl in my setup so I can't independently test it. If someone can independently test this and confirm that it works, then I'll be able to slip it into 2.5. Thanks in advance.

I don't know of anyone to try to have them test. If someone does test and find issues I'll fix the patch as required.

(In [6236]) closes #2413 and closes #2974 adding an optional low-res image and DID information
