prefix tag for call queue is added multiple times when looping

Description

I have several call queues defined.

In one of them, I set a prefix of "XXX:" and that displays fine when the call is ringing all the agents in the queue. This queue times out and directs to voicemail if there is no answer during business hours.

I have a second queue, with the prefix "YYY:", and this displays fine on the first rotation through the queue. After it times out, this queue loops back to itself. When it loops back, the "YYY:" prefix is added again, so the callerid data now looks like "YYY:YYY:<name>".

There is code in the ring-groups dialplan logic to only add the prefix for a ring group if it doesn't already exist. This same logic should apply to call queues.
