Voicemail menu is not hidden when not enabled

Description

This is a regression twards freePBX 2.1 and AMP:

When the module "voicemail" is installed, and the user disables the menu-

On old installations of freepbx/amp when the user selected not to use voicemail for an extension, that part of the gui was hidden with some JS mojo. Now it's always visible.

I consider this a regression in the UI which must be addressed.

Fixing should be "trivial", and IMHO can be applied to the branch and not only trunk.

I think it was a result of voicemail being pulled out as a module and the guielements ... code... was put in.

will address with quickform in next release when all the sections should get more control as far as being visible or not.
